List of documents required for postpaid subscription

Prior to signing the Contract, the Subscriber shall provide the Operator with required personal data and copies of the following documents:

For resident legal entities:

• state registration certificate;
• VAT payer certificate (if available);
• single-tax payer certificate (if available);
• tax-payer registration certificate (form No. 4-ОПП);
• abstract from the Unified State Register of Companies and Organizations of Ukraine;
• document confirming the person’s authority to sign the Contract (excerpt from the legal entity’s Articles, etc.);
• bank details;
• information on the registration and actual location addresses (mailing address);
• contact person’s details and contact telephone number in Ukraine.

If the Contract is signed by a branch office or a representative office, the documents to be submitted shall also include an excerpt from the Regulation on the Branch Office/Representative Office that should provide for the right of this particular structural unit to sign telecommunication services contracts, or a power of attorney giving the authority to the official of the branch office/representative office to sign telecommunication services contracts.    

 

For non-resident legal entities:

A contract with non-resident foreign entities may only be signed provided they have an official representative office in Ukraine. The required document list is the same as the one determined for resident legal entities.
The documents specified shall bear the legal entity’s seal and the inscription “True copy”, as well as the manager’s (authorized person’s) signature. Notarized document copies may also be used.
The Contract and corresponding documents shall be signed by the legal entity’s manager and sealed on behalf of the legal entity. If the documents are not submitted by the manager, a corresponding power of attorney is required for signing the Contract and representing the legal entity’s interests before the Operator, as well as the authorized person’s passport.       
To receive the Subscriber Equipment paid for by a cashless bank transfer, a corresponding power of attorney issued according to the form prescribed by the law should be presented along with the authorized person’s passport.

For resident physical persons:

• valid passport of the citizen of Ukraine having a note of the bearer’s registration in Ukraine;
• address of residence (mailing address);
• contact telephone number in Ukraine;
• personal identification number certificate.

For non-resident physical persons:

• valid passport of the citizen of a foreign country having a note of the bearer’s registration at the point of entry to Ukraine, and a personal identification document for stateless persons;
• permanent or temporary address of residence (mailing address) in Ukraine;
• contact telephone number in Ukraine;
• personal identification number certificate (if available).
